Animal Control Forms

Nuisance Complaint Form (.pdf)
Nuisance complaints can be filled out and submitted IN PERSON ONLY to Animal Control. All nuisance complaints MUST be signed in the presence of one of the Animal Control Officers on duty and ONLY signed by the person presenting such complaint.

Pet Adoption Application (.pdf)
Adoption applications can be filled out and submitted to animal control by mail, email (animalcontrol@smithfieldpd.com) or in person.

Pet Adoption Information

An adoption fee is required:

Cats ($95)
Male Dogs ($125)
Female Dogs ($150).

You also must sign the following documents:

Fill out a Pet Adoption Agreement at the shelter.
Rabies vaccination is required within fourteen (14) days;
Agree to have the animal spayed or neutered within thirty (30) days. (Per state law).
