What can I do if I feel I have been discriminated against by this agency, based on race, color, national origin, religion, sex, disability, or age, in its delivery of services or benefits?

You may file a Civilian Complaint with the Smithfield Police Department; and/or report the alleged discrimination to the Rhode Island Public Safety Grant Administration Office (RIPSGAO), 311 Danielson Pike, North Scituate, RI 02857 (401-764-5796); and/or report it to the Office of Civil Rights (OCR), U.S. Department of Health & Human Services, at www.hhs.gov.
